[pwc] bugs ?

Hello,

I fill this bug in the kernel bugzilla :

Basicaly,  i use a pwc camera using mmap in a software . I launch this
software inside gdb, and i kill the soft in gdb. Then it's impossible
to reopen the device. When i watch the trace, .release method is never
called, so the /dev/video0 is locked until the next reboot.

I don't think it's a pwc bug, but a kernel bug.  If you can reproduice
this, this could help the kernel developpement.

Beside  that, <sys/module.h> miss inside pwc-ioctl.h to be used
directly (compiler complain about _IOW).
